The hearing itself, transcribed with consistent attribution of chair, presenting officer, employee and companion.

The complaint as the employee actually stated it, and the employer’s response, recorded without paraphrase.

Appeals
A record of the appeal that can be read against the original hearing, so differences of account are visible.
Meetings are transcribed from recordings made by the organisation, or documented live by electronic notetaking where recording is not appropriate. Either way, distribution is controlled: transcripts are delivered to named recipients only, under arrangements agreed in advance.
